Care Navigation
Our Care Navigation Program offers a comprehensive Care Plan catered towards each of our clients, allowing them to find the proper care for their specific needs.
Hospice
Our Hospice program provides support and guidance for terminally ill clients over 18 and their families, so that each client can live as fully and comfortably as possible.
Bereavement
We offer a multitude of grief support programs for all ages, from our CHANGES family grief program to our Journey adult grief support groups to our school-based bereavement program.
Quality care at no cost.
All of our services at Pathways come at no cost to our clients or their families. We have been a reassuring presence in the Greater Lakewood/Long Beach area for nearly 40 years, helping hundreds of thousands endure the most difficult moments of their lives.

Here at Pathways, we believe in promoting independence and self-esteem for older adults, enhancing the quality of life for the terminally ill, providing assistance to families in grief before and after the death of a loved one, and supporting grieving children through special programs.
Pathways Honored at
Lakewood Celebrates
In July 2023, Pathways had the privilege of being honored at Lakewood Celebrates, an event held by the city of Lakewood to honor members and organizations of the community that have been performing outstanding work. Volunteers Alma Egan and Margie Garcia were given certificates by the city to honor their work as Pathways’ Volunteers of the Year in both our Care Navigation and Bereavement programs.
